Clinical Repurposing Research to Impact Veterans/Military

Cures Within Reach

This Request for Proposals (RFP) from Cures Within Reach (CWR) is seeking clinical repurposing trials from US-based research institutions using already approved treatments to address unmet and high-priority medical needs of US veterans and/or active US military, such as mental health, post-traumatic stress disorder (PTSD) and brain injuries. Our strongest preference is for proof of concept, pivotal, Phase I or Phase IIA clinical repurposing trials. Eligible trials may be conducted at accredited US-based academic, nonprofit and governmental research institutions and/or health systems involved with medical research, including Veterans Administration (VA) health systems and hospitals.

Repurposed therapies must be approved and/or generally recognized as safe for human use by some regulatory agency, such as the US Food and Drug Administration, European Medicines Agency, Health Canada or Japan’s Pharmaceuticals and Medical Devices Agency, and may include drugs, devices, nutraceuticals or diagnostics, as well as combination therapies. These repurposed therapies must be tested in a new indication for which they are not already approved or used widely in clinical practice as standard of care. Repurposed therapies also may be added to current standard of care to improve patient outcomes and/or quality of life.

CWR is accepting budgets of up to $75,000 for this funding opportunity, which includes the required institutional match (see Important Funding Information below). The total funding amount submitted to CWR must be sole, late or final funding required to accomplish the specific aims listed in the Letter of Intent (LOI). For Principal Investigators (PIs) who are interested in including underrepresented patients in the proposed clinical trial, CWR may provide $5,000 - $15,000 in additional financial support for approved community engagement costs. Community engagement must go beyond patient engagement and include collaborations with community-based organizations (i.e. community health, social services and/or faith-based organizations – including veterans-focused groups). Community engagement is required for any trials addressing health disparities and is optional for all other trials.

AI Based Application Success Predictor

Based on funded projects and evaluation criteria:

🥇 1. Clear Drug Repurposing Strategy (Top Predictor)

  • Must involve:
    • Existing drug → new indication

✔ Strong proposals:

  • Identify a specific drug with rationale for new use

❌ Not eligible:

  • New drug discovery projects

🥈 2. Clinical-Stage Readiness

  • Preference for:
    • Projects ready for human trials

✔ High success:

  • Phase II or proof-of-concept studies

🥉 3. Strong Biological Rationale

  • Must explain:
    • Mechanism of action
    • Why the drug will work in the new condition

✔ Mechanistic justification is critical

4. Existing Safety Data Advantage

  • Key strength of repurposing:
    • Known safety profile

✔ Predictor:

  • Drugs with established human safety data

5. Feasibility & Speed

  • CWR prioritizes:
    • Projects that can deliver results quickly

✔ Winning proposals:

  • Short timelines
  • Clear milestones

6. High Patient Impact

  • Strong emphasis on:
    • Unmet medical needs
    • Rare or underserved diseases

✔ Predictor:

  • Potential for rapid clinical benefit

7. Cost-Effectiveness

  • Preference for:
    • Low-cost interventions
    • Scalable solutions

8. Translational / Clinical Outcomes

  • Must include:
    • Measurable clinical endpoints

✔ Example:

  • Symptom improvement
  • Biomarker changes

9. Intellectual Property (IP) Considerations

  • Less emphasis on:
    • Patent novelty
  • More focus on:
    • patient benefit and speed

🧪 Common Funded Research Areas

  • Rare diseases
  • Oncology (drug repositioning)
  • Neurological disorders
  • Infectious diseases
  • Chronic inflammatory conditions

⚠️ Common Reasons for Rejection

  • No clear repurposing angle
  • Early-stage/basic research only
  • Weak biological rationale
  • Not clinically ready
  • Overly complex or long timelines

🧠 Bottom Line (Key Predictors Ranked)

  1. Clear drug repurposing concept
  2. Clinical-stage readiness
  3. Strong biological rationale
  4. Feasibility and fast execution
  5. High patient impact
  6. Use of existing safety data

• Be interventional clinical trials testing repurposed therapies, either alone or in combination with standard of care or other repurposed therapies, in a new indication to address unsolved diseases facing retired US military veterans and active US military members

• Be conducted at any US-based accredited academic, veteran nonprofit and governmental research institutions and/or health systems significantly involved with medical research, including VA health systems and hospitals, where good scientific research and clinical practices can be assured

o Although start-ups, biotechs, pharmaceutical companies and medical device companies are not eligible for this RFP, their collaborators at eligible institutions can apply

• Have a timeline of no longer than 36 months

• Be led either by a previously funded PI or by an early-stage investigator who has received little or no extramural research funding to date

o PIs who have not received extramural funding previously and/or who do not currently have their own lab should include a Letter of Support from a funded, senior researcher who will act as a mentor for the proposed research and the PI

• Not yet have funding (see Important Funding Information below) or already have funding from another source, when funding from CWR will help improve the chances of success of the project and/or help speed patient impact
