Hand & Microsurgery Clinical Fellowship Programme

This programme is designed to give you a meaningful, hands-on exposure to hand and microsurgery, with a strong focus on clinical judgement, and integrated multidisciplinary care. Over the course of your attachment, you will be involved in a variety of cases, offering insight into both the technical and decision-making aspects of this highly specialised field.


While the clinical pace work may be demanding, we encourage you to take ownership of your learning by asking questions and engaging actively with all members of the team (nurses, therapists) to gain a holistic experience.


Beyond clinical skills, we hope you will also gain an understanding of the philosophy of hand surgery, from preoperative planning to communication, teamwork and therapy. 

Programme objectives: 

  • To impart knowledge on the art and science of Hand Surgery.
  • To educate surgeons (orthopaedic, plastic, or general) in the basic and finer points of the practice of Hand and Microsurgery.
  • To spread the philosophy and establish a network of Hand Surgeons in the region.

Main Areas of Interest

Syllabus: 

  • Consists of, but is not exclusive to, the following main topics in Hand Surgery:
  • Congenital: Syndactyly, polydactyly, brachydactyly, Apert’s, Radial club hand.
  • Infective: Bacterial, viral & fungal infections of the nails, digits, hands, and forearm.
  • Inflammatory: Rheumatoid hand, psoriatic arthropathy, gouty arthropathy.
  • Traumatic: Fractures of all the bones in the upper limb, soft tissue injuries to the tendon, nerves, vessels, and connective tissues.
  • Metabolic: Gout, diabetes, thyroid disorders, parathyroid disorders.
  • Nerve: Brachial plexus, nerve compressions, injuries and loss.
  • Tumours: All benign and some malignant tumours of the bone, cartilage, muscle,nerves, vessels, and connective tissues found in the upper limb.
  • Degenerative: Loss of cartilage due to trauma, arthritides, inflammation or tumour and the replacement as well as reconstructive options.

1. Joint replacement of small joints of hand

  • CMC joint of thumb
  • PIP joints of fingers
  • MCP joints of fingers

2. Joint replacement of wrist joint

  • Wrist joints: Total and partial
  • Distal radioulnar joint: Total and partial

3. Joint replacement of elbow joints

  • Proximal radius  
  • Total elbow replacement

4.Small joint arthroscopy

  • Wrist, elbow, CMC joints

5. Endoscopic nerve release

  • Carpal cubital tunnel release

6. Internal fixation of upper limb with use of 3D planning for corrective osteotomies, complex fractures, patient-specific instrumentations

7. Congenital hand problems

  • Treatment including corrective surgery and toe transfers

8. Microsurgery involving replantation, skin flaps, muscle flaps

9. Nerve and tendon repairs including microsurgical repair and reconstructions involving nerve transfers

 

The Clinical Attachment Programme is divided into clinical and academic components, providing candidates with the opportunity to actively participate and learn from them. A typical week includes a good mix of accompanying the attending consultant in the operating room, outpatient clinics, didactic sessions, and research hours. Candidates are expected to participate in case discussions and CME sessions. Candidates will also have on-call responsibilities, providing emergency care and surgical intervention under the supervision of senior surgeons.

1. Malaysian Medical Council (MMC) / Temporary Practising Certificate (TPC) – Fee Structure
For non-Malaysian applicants applying through the Malaysian Medical Council:

  • Fee of MYR 500 (per certificate) shall be made for each application

2. Expected Timeline

A complete application shall be submitted at least six (6) weeks prior to the commencement of intended practice.

The typical processing timelines are as follows:

  • MMC processing: approximately 4–8 weeks (with possible extension up to 10–12 weeks)
  • Additional time for queries/conditional approvals: ~2–3 weeks

3. Programme Structuring
To ensure compliance and avoid any regulatory ambiguity for international applicants:

Track 1 (No TPC Required):

  • Observership / Visiting Fellowships
  • No hands-on exposure/ scrub and stand-by only
  • Can commence immediately

Track 2 (TPC-Based):

  • Clinical fellowship with supervised hands-on exposure
  • Commencement subject to MMC approval and issuance of TPC

 

Candidate should self-arrange their own accommodation.

Please note that all TPC applications will be submitted by the candidate’s fellowship supervisor at Pantai Hospital Kuala Lumpur, who is a registered medical practitioner with a valid Annual Practising Certificate and will assume responsibility for the applicant.

What is primary source verification?

Primary source verification is a procedure to confirm authenticity of a qualification directly with the awarding institution via a third party.
As a regulatory body, it is our utmost priority to safeguard and provide assurance to the public that the medical practitioners registered with MMC are qualified and competent to provide the highest standards of medical care. It is a best practice against fraudulent credentials.
a. Medical graduates including Malaysians who graduated abroad applying for provisional or full registration for the first time b. Doctors with foreign postgraduate qualifications/credentials applying for specialist registration c. Doctors with foreign qualifications applying for temporary practicing certificate (TPC) for the first time
a. Local graduates from Malaysia b. Doctors with local postgraduate qualifications applying for specialist registration c. Non-Malaysian doctors applying for renewal of full registration d. Doctors who possess a prior registration with the MMC using the same qualification e.g TPC
Currently, the MMC have appointed 2 bodies for the verification of medical credentials. The applicants may choose any of these verification body, and application should be made in a timely manner to ensure that the application can be processed on time. a. Educational Commission for Foreign Medical Graduates (ECFMG) through their online system Electronic Portfolio for International Credentials (EPIC) b. The DataFlow Group through their online system – TrueProfile.io
a) Before you start your verification process, you should check whether your degree is listed in the Second Schedule and/or your postgraduate qualification is listed in the recognized postgraduate medical qualifications. If your qualification do not meet the registration criteria, the MMC will not grant you with a registration, even if they have been verified. b) Set up an online account with either EPIC or TrueProfile.io c) Upload the required qualification(s) for verification in the online account d) Once your qualification has been verified, the ECFMG or the DataFlow group will notify you and MMC. The verification report will be sent to MMC directly. e) Once verified, your completed application will be processed.
You are advised to check the fee section of the ECFMG/ DataFlow Group as all verification payments will be made directly to them. Please be reminded that all cost for PSV is at your own expense.
This depends on a number of factors, including: - the time it takes for your awarding institution to respond to - whether your awarding institution accepts these requests electronically (sending requests by post or courier will take a longer time). We recommend that you start the verification process as early as possible and have your qualifications verified before applying for a registration with us.
