Antiretroviral therapy (ART) has transformed HIV into a manageable chronic condition. Yet, people with HIV face high risk and early onset of comorbidities, ART-related adverse effects and interconnected medical, psychosocial, and structural challenges. Across the lifespan, HIV and its treatment may also affect individuals exposed to HIV, e.g. HIV-exposed, uninfected children may have elevated risks of immune, metabolic, and developmental outcomes compared with HIV unexposed, uninfected peers, particularly in low and middle-income settings. The broad spectrum of HIV-associated co-occurring conditions comorbidities, coinfections, and complications, including mental health and substance use disorders, shaped by social determinants of health may benefit from interdisciplinary approaches to care and research.

Key gaps remain in understanding how HIV, ART, and psychosocial stressors interact to drive early onset and progression of comorbidities. Many comorbidities involve intersecting pathways and share risk factors even as they diverge into disease-specific trajectories, e.g., metabolic syndrome increases risk for diabetes and cardiovascular and chronic kidney diseases, while certain coinfections increase cancer risk. Elucidating shared factors may inform preventive strategies that address multiple outcomes.

Traditional disease-specific models may insufficiently capture shared factors or multilevel implementation determinants. Comprehensive, whole-person care approaches linking multiple disciplines such as infectious diseases, endocrinology, psychiatry, nursing, pharmacy, and community health, with emphasis on early screening, lifestyle-based prevention, and sustained engagement in HIV care may improve health and quality of life (QoL) of people with HIV, while requiring careful consideration of local context and health system factors for effective implementation.

This topic promotes interdisciplinary collaborative science integrating biological, behavioral, social, population, and implementation approaches to understand and improve health of people with HIV. Multidisciplinary teams bridging basic, clinical and implementation science expertise with meaningful community engagement are strongly encouraged.
